NHS Certificates (Sick Line/Note)

• If you have already visited the Doctor and been issued with a sick note you may not need to make another appointment to have this reissued.  Please leave a message for Reception who will pass on the request to the Doctor 

• Please try to avoid telephoning too far in advance as we cannot forward date a certificate 

• You do not need a Doctor's Certificate unless you have been absent from work for 7 days.  For this you can self-certify your absence 

• If your employer insists on a certificate THEY may contact us (with your written consent) and we can supply a private certificate for which they will have to pay a fee

• You no longer require to be signed back to work after an absence

• Please note, we do not issue letters or certificates for school/exam absence due to illness
